• Jarisch Herxheimer reactions are usually not life-threatening.
• Prednisone is also used for the treatment of the Herxheimer reaction, which is common during the treatment of syphilis.
• Initially, antibiotics may cause a worsening of symptoms ( the Herxheimer reaction ), as spirochaetes are destroyed, which in part supports the diagnosis.
• As with antibiotic therapy and other methods of countering bacterial infections, endotoxins are released by the bacteria as they are destroyed within the patient ( Herxheimer reaction ).
• These can, however, induce a Jarisch Herxheimer reaction in over half those treated, producing anxiety, diaphoresis, fever, tachycardia and tachypnea with an initial pressor response followed rapidly by hypotension.
